Arizant reserves the right to reject my submission, or to edit/delete any parts of it that violate these Terms and Conditions. Furthermore, I understand and agree that the consent for my testimonial cannot be revoked and that all original material submitted will be the sole property of the Arizant family of companies. Testimonials and photographs submitted must not contain any personal medical data or any protected health information about a patient, as defined by the Health Information Portability and Accountability Act of 1996 (HIPAA) Privacy Rule. In the event I do disclose individually identifiable protected health information about myself, I agree to Arizant’s use and disclosure of such information as described in these Terms and Conditions. I understand that if my individually identifiable protected health information is disclosed to someone who is not required to comply with U.S. Federal privacy laws, such information may be re-disclosed and would no longer be protected by U.S. Federal privacy laws.
